Dr. Jay Hazen is a 1991 graduate of the Marquette University Dental School. He has several hundred hours of continuing education credits in all phases of dentistry including intensive training and education in aesthetic and reconstructive dentistry and is a current member of the following:

Dr. Tamsen is a native of Cudahy, WI and a 2017 honors graduate from Marquette University School of Dentistry. Prior to dental school, Dr. Tamsen earned his bachelor's degree in Medical Technology from Marquette University and served U.S. veterans as a medical technologist at the Milwaukee VA Medical Center. Like Dr. Hazen, Dr. Tamsen is dedicated to providing the same friendly, professional, and comfortable care patients have come to expect at Dentistry For Madison.

I have been a part of the Dentistry For Madison team since graduating from the MATC program in 1996. I love that the world of dentistry is always changing and improving and our practice is changing and improving along with it. I think that our patients are impressed with how new technology is in our practice and it makes it much more enjoyable. I like being a dental hygienist because it is not only about teeth, but also about the relationships formed with our patients. I also think educating our patients is key. Patients are amazed at how interesting their mouths are!
I am a graduate of the dental hygiene program from MATC and have been practicing at Dentistry For Madison since 1996. What keeps my job exciting is the technology and treatment that we have available to our patients. Our office is committed to offering the best possible treatment in an atmosphere that reassures the patient that he/she is getting the best care possible.
I graduated from the MATC Dental Hygiene Program in 1999. In 2004, I jumped at the opportunity to become part of the Dentistry For Madison team, and haven’t looked back since. I love what I do, and love our patients. Our staff has truly become a family over the years. We work well together, and are committed to offering the best service possible. Quality care, with the latest technology, in a comfortable environment is what we are all about.
Dentistry is a family affair. I have twin brothers who are orthodontists and a sister-in-law who is a hygienist. With dentistry as a family trait, I started my career as a dental lab technician, and worked my way into dental assisting. Realizing I wanted to be more involved with my patients prompted me to go back to school to become a dental hygienist. Being intrigued with the upbeat atmosphere and advanced technology at Dentistry For Madison, I started working with them in April 2007.
I enjoy being a team member and love the relationships I have made with our patients. Being a hygienist, along with my Natural Health bachelor studies is very rewarding. It enables me to help people with their great smiles and overall health. My interests include sports, music, fishing, camping, boating and cycling.
I graduated from Miami Dade College Dental Hygiene Program 30 years ago with a BS in Health Care Administration and MS in Health Literacy. I’ve taught at Madison College in the Dental Hygiene Program and at Marquette University's Dental School as an Assistant Clinical Professor. In my spare time I’ve volunteered at Madison Dental Initiative, Mission of Mercy and Madison Literacy Network and have been a freelance peer reviewer for P&G Dental Hygiene Continuing Education. I enjoy working at Dentistry for Madison with Dr. Hazen and Dr. Karls, not only because of the high quality dentistry they provide to their patients, but because of the cutting edge technology the office uses. This high level of quality care combined with state-of-the-art dental technology gives our patients the very best dental care possible!
I graduated from the Dental Assisting program at Madison College in December 2014 and have been with the Dentistry for Madison team since February 2016. I am honored to work with such an amazing group of people, who I call family. I have a passion for what I do because it is so incredible to make such a difference in someone's life by improving their smile.
I have been a part of the Dentistry for Madison team for a total of 15 years and I can honestly say I love what I do. Not only assisting the doctors with all of our awesome patients and always learning something new, but also getting to work with a great group of people. We truly are a family here at Dentistry for Madison, the employees, and patients.
I completed the dental assisting program in 2012 and recently joined Dentistry For Madison in September of 2018. Thus far it has been amazing to work with such a great group of people. I love assisting because the gratitude I get with helping someone improve their smile is amazing.
